Charitable Donation Request Form
Any individual seeking a donation from the California Baptist University Athletics Department must first complete and submit the following Charitable Donation Request Form. The CBU Athletics Department will review the completed form and any additional documentation submitted. Requests must be completed in its entirety and submitted a minimum of four weeks prior to the requested deadline.
